Are you passionate about patient safety in our communities? We’re looking for an expert in healthcare facility inspections and investigations to join our team!
This position is a home-based, hybrid, full-timeHealth Services Consultant 4 position located in the Office of Health Systems Oversight within the Health Systems Quality Assurance Division.
The position’s work requires regular in-person attendance to perform duties at various locations throughout Washington state, the official duty station for this position is home-based.
What You Will Do:
As a member of the In-Home Services Inspection and Investigations Team, Health Services Consultant 4 (HSC 4), you will:
Conduct on-site and off-site inspections and investigations in multiple geographic areas within Washington State to determine compliance with state and federal health and safety standards and regulations in assigned health care facilities including but not limited to: In-Home Service Agencies (IHS), Home Health, Home Care, Hospice, and Rural Health Clinics applying professional judgement, skills, and technical knowledge.
Expectations and Responsibilities:
Lead and direct initial and routine inspections and/or complaint investigation coordination of assigned facilities in the direction of the supervisor.
Develop an overall approach and create an inspection or investigation plan by gathering information on past inspections, complaints, current laws and rules, team member/co-worker input for planning and organizing inspection or investigation processes.
Collect information by interviewing staff, residents, family members or representatives, providers, agency/facility personnel, other interested or affected parties, and review documents, policies, and procedures.
Observe and evaluate the health care facility or agency treatment program as well as operation of facility health and safety activities during inspections and investigations for compliance with health and safety standards.
Use environmental measurement instruments to collect information about the facility and observe environments for safety hazards including slip hazards, air quality, and hygiene, infection control, food safety, and physical environment.
Review policies and procedures to evaluate the regulated health care facility or program compliance with laws, rules, and regulations.
Review health care facility staff or personnel records to evaluate compliance with required personnel requirements.
Identify findings in state and federal inspections and investigations to compile and prepare necessary documentation of deficiencies or violations in support of proposed corrective or legal action.
Serve as primary point of contact with the facility representative and Office of Health Systems Oversight management during an inspection or investigation.

Location and Flexibility
Enjoy the flexibility of our hybrid work environment. You’ll balance working from home and performing work activities in the field in communities in multiple geographic areas within Washington State. This position interacts with other inspectors and investigators in the office, facility staff, patients, relatives, other agency and government staff, local and national health care professionals, and the public.
Required Qualifications (you must meet ONE of the provided options and any additional criteria listed.):
Experience defined below may have been gained through paid or unpaid activities. Please ensure any relevant experience defined below is outlined in your cover letter, resume, and/or applicant profile.
Six (6) or more years of professional experience in applying state and federal health and safety standards and regulations in a survey, inspection, investigation, auditing, or accreditation environment or experience working in social work, social science, speech pathology, occupational or physical therapy, psychology, public health, health care administration, public or private sector health-related industry, or in an investigative, regulatory, or other related field; or licensed as a Midwife in the State of Washington or eligible to obtain licensure within six (6) months of hire.
OR
Abachelor’s degree or higher in social work, social science, speech pathology, occupational or physical therapy, psychology, public health, health care administration, or another related field.
And
Two (2) or more years of professional experience in applying state and federal health and safety standards and regulations in a survey, inspection, investigation, auditing, or accreditation environment or in a public or private sector health-related industry, or in an investigative, regulatory, or other related field.

Observation & Inspection:
Uses visual inspection and pays close attention to details.
Recognizes and responds to details that are easy to overlook or dismiss as insignificant.
Uses informed judgment, recognizes, or anticipates when deviations from expected observations indicate a problem or issue that must be corrected.
Identifies the most appropriate course of action in response to the observations, or to correct the situation.
Is able to distinguish which critical observations must be documented.
Understands how to document critical observations in a way that others can understand and act upon their significance.
Preferred Qualifications:
Experience in environmental health or registered as an Environmental Health Specialist or Registered Sanitarian.
Experience working in health care settings such as: In-Home Services, Home Health, Home Care, Hospice, Long-term Care settings, and/or Rural Health Clinics.
A master's degree in social work, social science, speech pathology, occupational or physical therapy, psychology, public health, health care administration, or other related field.
Previous demonstrated experience in a regulatory, survey, inspection, investigation, or accreditation environment. Experience working with and interpreting state laws, or federal regulations related to In-Home Services, Home Health, Home Care, Hospice, or other regulated licensed or certified healthcare facilities.
Knowledge of principles, practices, organization, and objectives of environmental health; statutes, ordinances and resolutions pertaining to environmental health; state and federal laws and State Board of Health (SBOH) regulations relating to environmental health; bacteriology; vector control; and educational principles and techniques.
